An HMO is a Health Maintenance Organization. An HMO is an organized system that provides a wide range of health care services, including Primary Care Providers (PCP) to "manage" health care needs. Each HMO uses different hospitals, pharmacies and doctors for the counties it serves.

For those clients that are part of or applying for CHP+ Prenatal benefits or reside in El Paso, Sedgwick or Teller Counties please refer to the State Managed Care Network information below.
For clients that are participating in the CHP+ PE program please refer to the State Managed Care Network information below.
For all other clients please look at the HMO Map and HMO Chart to review what HMOs are available in your county. Please remember that an HMO MUST be selected on your application.

Effective March 1, 2011 Colorado Choice is now available to our clients in Baca, Cheyenne, Kiowa, Kit Carson and Washington counties.
