HMO Availability and Comparison

Effective 08/01/09 Rocky Mountain Health Plans is now available as an HMO in La Plata and Montezuma counties.
Effective 09/01/09 Rocky Mountain Health Plans is now available as an HMO in Garfield County.

An HMO is a Health Maintenance Organization. An HMO is an organized system that provides a wide range of health care services, including Primary Care Providers (PCP) to "manage" health care needs. Each HMO uses different hospitals, pharmacies and doctors for the counties it serves.

If you live in a county where HMOs are available you must choose one. Find your county on the map below and click on it to read about the HMOs in your area.
